Accident/Incident Investigation

In its broadest sense, an “incident” is anything that was not supposed to happen. There is no other activity that produces faster OH&S results than promptly reporting and investigating incidents. For this reason, incident investigation is a vital part of every health and safety management system.

COURSE OVERVIEW

Investigating an incident involves much more than filling out a report form. Incident investigation is a process of gathering facts and drawing conclusions; the report form is simply the tool used to document this process.

The purpose of incident investigation is not to find fault or fix blame. The twin purposes of an incident investigation are to:

  • determine the causes of the incident, and
  • enact corrective measures to
    prevent a recurrence.

COURSE TOPICS

After completing this course, participants will be able to:

  • explain the purpose and benefits of incident investigations,
  • describe how to prepare for, and conduct, an investigation,
  • recognize and distinguish between immediate (direct) causes of incidents and underlying (root) causes, and
  • develop recommendations for
    corrective actions to prevent incidents from recurring. This course includes
    classroom instruction, group discussions, exercises, and an exam.

Testing conducted in this online course is designed to reinforce the information presented. A mark of 80% must be achieved in order to pass this course

CERTIFICATE OF COMPLETION

Upon successful completion of this online course, a certificate of completion will be available for download and printing. This certificate is valid for 3 years from the date of completion printed on the certificate unless otherwise specified by an employer, legislation, or other authority. This course is recommended to be retaken every 3 years from the course completion date indicated on the certificate.
